I've seen it at least once. A return was filed with a total income listed on the schedule C but the type of business said " refuse to answer on 5th amendment grounds". Turns out it was a drug dealer that got caught and didn't want a federal tax charge added to the drug charges. He didn't pay the tax due by the way....
