They do get creative.
One of my favorite excuses for not filing was from a federal court case several years ago. The taxpayer claimed he didn't file his income tax returns because he had a phobia of numbers. The problem with that theory, as pointed out by the judge, was that the taxpayer was a CPA and had operated his practice successfully during the entire time period.
Needless to say it didn't work...
